Remain Independent in Your Own Home The Reutlinger Community for Jewish Living is one of the outstanding senior living communities in Northern California. We recognize, however, that institutional care is not always the only option. There are many elderly who would be capable of continuing to live independently in their own homes if individualized support services were available. Since 1976, this need has been met through our Community Outreach Services Division. With today's multi-generational families many caregivers work full-time or are themselves facing the aging process, and many elderly have no family. Without assistance in their homes they are often prematurely hastened into communal living environments. Community Outreach Services extends the time some elderly can remain in their homes. These services also help families to alleviate the burden of providing assistance themselves. In keeping with our goal of enhancing the quality of life, the Community Outreach Services Division is dedicated to maintaining the comfort and dignity of the elderly in our community. Below is a listing of services through our Community Outreach Services division: |
| Care Management Service Through the assessment of needs and assistance with identifying options available, aide is given to foster independence. Implementation, ongoing monitoring, coordination, and follow-up are provided to families. |

| Out-of-Area Assistance Families separated from their elders by geography can receive a coordinated program of services and support, including case assessment and reports to family on a regular basis. Service and assistance is available in all 50 states. |

| Assessments Evaluation of needs and coordination of services are offered to help elders function independently. |

| Consultations We provide general assistance with long-term planning, placement, and information and referral. |

| Geriatric Counseling Our professional individual and family counseling (brief or long-term) works on relationship problems concerning older adults, provides resource information, and teaches how to cope with stress brought on by change or illness. |

| Home Health Service Personal care assistance provides help with medication, dressing, and bathing by licensed home health aides, vocational nurses, or registered nurses in conjunction with doctor's orders. The service includes individualized assessment of needs and maintenance of program monitoring. |

| Homemaker Services Shopping, cooking, cleaning, and transportation services are provided by experienced, bonded, and insured aides. Our social worker ensures appropriateness of service through follow-up and service coordination. |
